New Horizon Centres

The Alzheimer's Disease Association manages and supports the New Horizon Centres which provide day care services for the elderly with dementia. The New Horizon Centres at Toa Payoh and Bukit Batok also offer an Early Dementia Programme for those with early stage dementia. Click here to know more about the program. The New Horizon Centre (Tampines) is a day rehabilitation centre for people with strokes, fractures, Parkinson's Disease and related disorders who require follow-up care after their discharge from hospital.

 

Objectives:

bullet To provide relief for caregivers
bullet To increase families' abilities to cope with and continue caring for people with dementia and/or physical disabilities
bullet To provide nursing care, occupational therapy, physiotherapy and social activities
 
 

Professional staffing
The Centre's team of professionals includes doctors, occupational therapist, physiotherapist and nurses.


Activities/Programmes
Each individual is considered unique and valuable. Therefore, there is an individualised care plan for each person. Clients are carefully assessed to determine their physical, cognitive and social functioning in order to put together an effective therapeutic care plan.

Activities at the Centres include:
- occupational therapy and physiotherapy
- group exercises
- group and individual games
- reminiscence
- leisure and hobbies
- outings and excursions

Family involvement
The involvement of the family is crucial. Regular feedback is given to the family. Caregivers are encouraged to attend the meetings of the Family Caregivers Support Group where information and sharing on caring for a person with dementia are provided.

 

Transport
Transport will be provided for those living within 5 km of the Centre.

Charges
Attendance fees are charged based on household income.
  Lunch and two teas are provided. Transport charges are based on the distance between the client
's home and the Centre.

 

Criteria for Admission

bullet Diagnosis of dementia or related disorders with medical conditions that are stable and non-infectious
bullet Ambulant or semi-ambulant
bullet Clients with unmanageable or disruptive behaviours may be excluded
 

Application procedure
